Deviations, improvements, and customer feedback are registered to ensure that each safety incident, quality incident, improvement suggestion, observation, and customer feedback is recorded and handled with high quality. Furthermore, provision is made for monitoring and reduction of risk, improvement, and learning. The following pages will explain each type of registration and its respective form.
Anonymous reporting is separate from normal reporting on the use of security and compliance aspects for reporting related to criticism.
Things to Consider
Before you start using the QHSE reporting function, it is important that you have thought through some topics. To get started, we have prepared a list of questions you should consider before implementing QHSE reporting in your company.
- Will you use all available registration types?
- Can you define all the roles needed? (One person can have multiple roles)
- What is the minimum information needed for a new registration?
- Can you describe the minimum for each type?
- You know all the roles, but do they know what they should do?
- Have you defined deadlines for case handling, evaluation, and closing of the case?
- Can these deadlines vary between the different registration types?
- When you classify a registration, do you have a definition for the following options?
- Direct cause
- Original cause
- Category (normally not changed from standard, changes also affect reports)
- Subcategory (normally not changed from standard, changes also affect reports)
- For each impact category (health, finance, reputation, and environment) Do you have a clear definition of what this means in your company if you assess each category from 1 to 5? (1 = insignificant and 5 = catastrophic)
- How will you use measurement tasks? (corrective or preventive tasks related to the reported registration)
- Is there a reason for your company to create a workspace for a QHSE registration?
- Will we track costs? If yes, how should it be recorded?
- How do we ensure that our company learns from our registrations?
- Do we need anonymous reporting? If yes, how should this be followed up by nominated parties in the company?
There is no right or wrong. The questions above are intended to give you an idea of the necessary information you need to effectively implement a robust reporting system in your company.
The tip here is to document the handling process in a procedure that provides contextual meaning for your colleagues, managers, and other stakeholders.
If you choose to skip documenting the process, you risk ending up with a reporting archive with less transfer learning, with no or low closure frequency, or poor quality of results.
